Medical Clinic New Patient
Powered by Ryvet
Medical clinic intake covers presenting concern, medical history, current medications, and consent — and getting it wrong delays the consult. This template runs the intake before the appointment, in conversation, so the GP walks into the consult with a real history and an idea of why the patient is there. Reception spends less time chasing paperwork; the patient spends less time in the waiting room.
Most medical practices use paper or PDF intake forms that patients are expected to fill out in the waiting room. The result: rushed forms, missing information, and a GP who has to ask the same history questions in the consult, eating into the 15-minute appointment.
Doing intake the day before — via chat — means the patient takes their time, the data lands in the EMR, and the consult opens with 'I see you're here about the persistent cough you mentioned' instead of 'what brings you in?'. That's a real time saving and a better consult.
The chat asks why the patient is coming in, with enough detail to triage urgency without diagnosing. Mental health concerns, child consults, and chronic conditions get flagged for longer appointment slots.
Current medications, allergies, conditions, and recent surgeries are captured in plain language. The GP sees a clean history rather than a half-filled form.
Medicare card, private health, and consent forms are completed in the same flow. No 'just sign this when you come in' adding minutes to the front-desk handover.
GP clinics, medical centers, specialist consultancies (cardiology, dermatology, etc.), telehealth providers, and skin-cancer clinics handling new-patient intake at scale.
Yes — patient and intake data sync with most major Australian and international practice management systems. Custom field mapping per practice.
Data is encrypted at rest and in transit. Hosting can be configured to meet Australian Privacy Principles, My Health Records guidelines, or HIPAA depending on jurisdiction.
The chat is fully accessible (large text, screen-reader friendly) and family members can complete it on the patient's behalf via a shared link. For patients without internet, paper backup remains.
